Checking your benefits
As a patient you are responsible for knowing your own benefits and coverage before your visit. Your plan may or may not cover certain therapies, procedures or services from our providers; if you are coming in for a specific treatment, please confirm what it will cost you.
We may be able to verify certain benefits in the office using the same online tools available to you, but this depends on staffing and is offered only as a courtesy.

Any information about benefits and coverage we relay to you is not a guarantee of coverage by your insurance company and does not obligate us. You are ultimately responsible for your care and the fees for services provided to you.
Cash rates
If you do not have insurance coverage, these rates are payable by cash, check or credit card. They are a pay-at-service discount and do not represent the price if billed through insurance. PRP pricing depends on the preparation: Fidia single-spin, Arthrex double-spin, or EMCYTE.

Cancellation policy
We reserve the right to charge a $75 fee if you do not show up for an appointment or cancel with less than 24 hours’ notice.
An appointment cancelled at short notice could very likely have gone to another patient. We understand that emergencies happen and that schedules are sometimes tight, but repeated late cancellations will be charged. Three late cancellations or no-shows in a row may result in the loss of scheduling privileges.
If you are signed up for email reminders you will receive one a week before and one the day before your appointment. Otherwise, we try to give a courtesy call the day before.
